Beneport-GISC requires that every Remote User have necessary functionality and security precautions in place, as well as proper equipment and security training.

Do you use/have an extra monitor at you remote location?

Please select the type of internet service you currently have:
Is there an available Ethernet port on your home modem that you can plug your computer into directly?
Are you able to have conduct business within 20 feet of your home modem/router?

Please confirm if you already have a working CAT6 ethernet cable. Or if you'd need one, tell us the length of cable you need, in ft. 

(Not: Remote Users are REQUIRED to directly be connected to the router and have WiFi disabled). 

Are you able to conduct business in a quiet, private area that allow you to respect HIPAA privacy and industy-standard security requirements?

Will there be other remote workers/students at home during your workday?
